Freedom of Access Violations
MEETING
Use this form as a guide in recording a meeting violations such as questionable executive sessions and inadequate notice for use in either the challenging the holding of a meeting or to support an argument for changing the law.

- Identify the government body or agency involved.
- Provide the location, date, and time of the meeting, and state whether the meeting was a regular meeting or specially called.
- How and when was notice of this meeting given?
- If specially called, did the notice state a purpose or agenda, and if so what was it?
- Was there a motion to go into executive session?
- Was there a vote on the motion?
- How was the topic of the executive session described in the motion or by the chair? (Be as exact as possible.)
- Did you object to the executive session, and if so, what did you say?
- Did anyone not a member of the body attend the executive session, and if so, who?
- What have you learned about the discussion that occurred in the exectuive session and the action, if any, taken? E.g. were any votes taken, straw or otherwise?
- Did the body take any action upon emerging from the executive session, and if so what was it and was it preceded by any discussion?
